"The shape of our democracy is the issue that affects every other issue" is pretty crappy phrasing and doesn't sound either clever or communicate anything clearly, but it does have substance. The point he was making was that the democratic system affects which issues are prioritised and how people have to campaign and get things done, and that's completely true.

Buttigieg didn't talk much about it once he pivoted to the center, but initially, a lot of his campaign was against the current democratic system with two parties, where the results don't really represent the overall state of the country, and where basically everything big can get easily blocked in congress. If he had stuck with that, maybe he would have been more successful.
